Mounjaro injection guide: where and how to inject the pen

Mounjaro tirzepatide by Eli Lilly

Inject Mounjaro once weekly under the skin of your abdomen, thigh, or upper arm — rotating sites each week. Here's the general technique, step by step.

Mounjaro is a once-weekly injection you give yourself under the skin — not into a vein or muscle. In the US it comes as a single-dose pen: a push-button auto-injector, pre-filled at your prescribed strength, with no needle to attach or see. (Some markets sell a multi-dose KwikPen instead, covered below.) Here’s where the injection goes, how the technique works step by step, and the storage and disposal rules that keep it safe.

Where to inject Mounjaro

Mounjaro goes into subcutaneous fat — the soft layer just under the skin — at one of three sites (⚠ verify descriptions against the Instructions for Use):

  • Abdomen (belly) — the most common spot. Stay at least 2 inches away from your navel in any direction. Most people find the stomach the easiest area to see and reach.
  • Front of the thigh — the fleshy area on the top and slightly outer part of the upper leg, between hip and knee.
  • Back of the upper arm — the fatty area behind and below the shoulder. It’s the hardest to reach yourself, so many people save it for when someone can help.

All three absorb the medicine equally well, so choice comes down to comfort and reach. Whichever area you pick, avoid skin that’s bruised, tender, hard, lumpy, scarred, or red. And rotate — repeatedly injecting the same patch can build up lipohypertrophy, thickened fatty lumps that make future doses absorb unevenly. More on the rotation pattern below.

How to inject Mounjaro step by step

This is the general once-weekly technique for the US single-dose pen. Your printed Instructions for Use is the authority on the details specific to your pen — read it the first time, and keep it for reference.

  1. Wash your hands with soap and water, and gather your pen and a sharps container. The single-dose pen has no separate needle to attach.
  2. Inspect the pen and medicine. Check the label and look through the pen window — the liquid should be clear and colorless. Don’t use it if it’s cloudy, discolored, has particles, or if the pen looks damaged.
  3. Let it reach room temperature. Injecting straight from the fridge stings more; many people take the pen out a few minutes ahead of time (⚠ verify any timing against the IFU). Never warm it in a microwave or hot water.
  4. Pick and clean a spot. Choose one of the three sites above, wipe it with an alcohol swab, and let it dry fully before injecting.
  5. Uncap and unlock. Pull the base cap straight off, then turn the lock ring to unlock the pen (⚠ verify the unlock step against the IFU). Don’t touch or press the base until you’re on the skin.
  6. Press flat against the skin. Hold the pen so its clear base sits flush against the injection site, then press down firmly to start the injection.
  7. Hold until it finishes. Keep pressing flat and still — you’ll hear a first click when it starts and a second click when it’s done, roughly 10 seconds later (⚠ verify the click/hold signal against the IFU). Lifting too soon can leave part of the dose behind.
  8. Check and dispose. Confirm the dose window shows the injection is complete, then drop the whole pen into your sharps container — not the trash.

If you use the KwikPen sold in some markets, the sequence is different: you attach a fresh screw-on pen needle, dial your prescribed dose, inject, and then remove and discard the needle after each dose — the pen itself holds several weeks of medicine. Have a pharmacist walk you through the first KwikPen injection.

Because Mounjaro is approved for type 2 diabetes and prescribed off-label for weight loss, that use — like every dosing decision — stays directed by your clinician. The technique on this page is the same either way; the dose and the plan are not something to self-adjust.

Rotating injection sites

Rotating does one job: it protects your skin. Inject the same square inch week after week and the tissue can thicken into lipohypertrophy — firm, rubbery lumps that both look different and absorb medicine less predictably, which can make your dose work less reliably.

A simple rotation that’s easy to remember:

  • Rotate between areas across weeks — for example, abdomen one week, thigh the next, upper arm the week after, then back to the abdomen.
  • Rotate within an area too. If you use the abdomen two weeks in a row, shift the exact spot by at least an inch, and switch sides of the navel.
  • Keep a simple log — a note on your phone or a mark on a calendar — so you’re not guessing where last week’s injection went.

The abdomen has the most usable real estate, which is why many people anchor there and just keep moving the spot around it. Whatever pattern you choose, the principle is the same: never two injections in the same place back to back.

Storing Mounjaro

Storage matters because heat and freezing both ruin tirzepatide. General guidance (⚠ verify every temperature and day figure against the label at pi.lilly.com/us/mounjaro-uspi.pdf):

  • Before first use: keep pens in the refrigerator at 36–46°F (⚠ verify). Don’t store them near the freezer coils.
  • At room temperature: a pen can stay out below 86°F for up to 21 days (⚠ verify window) — then discard it even if medicine remains.
  • Never freeze it. A pen that has been frozen is ruined and must be thrown away, even if it looks fine (⚠ verify).
  • Traveling: keep pens cool and out of direct sun — a cool bag without a frozen pack touching the pen. Don’t leave it in a hot car or checked luggage.
  • Always store the pen with the cap on, away from heat and light, and keep it out of reach of children.

Needles and sharps disposal

The US single-dose pen is a one-and-done auto-injector — one pen per weekly dose, with the needle hidden inside; the whole pen goes into the sharps container after use, and there’s no separate needle to change. If you use the multi-dose KwikPen, each injection uses a new, single-use pen needle — one needle per dose, never reused and never shared. A fresh needle is sharper, hurts less, and avoids the infection and blockage risks a dull, reused one carries.

Used pens and needles are biohazard sharps and can’t go in household trash or recycling. Handle them this way:

  • Drop each used pen or needle straight into an FDA-cleared sharps disposal container — a rigid, puncture-resistant, lidded container made for the purpose.
  • If you don’t have one on hand, a heavy-duty plastic container with a tight screw-on lid can work as a temporary stand-in, but a proper sharps container is the right tool.
  • Don’t overfill it, don’t recap needles by hand, and when it’s about three-quarters full, seal and dispose of it following your local guidelines — programs vary by state and city.

The FDA’s sharps guidance (linked in Sources) explains local disposal options, from drop-off sites to mail-back programs.

Common mistakes and when to call your clinician

A few errors come up again and again:

  • Injecting into muscle instead of fat. Mounjaro is subcutaneous. Pressing the single-dose pen into a very lean spot, or using the wrong angle with a KwikPen, can go too deep — it stings more and can change absorption. If you’re lean, favor the abdomen or pinch a fold of skin where the IFU allows.
  • Lifting the pen too early. Keep the single-dose pen flat against the skin until both clicks finish, or you can leave part of the dose behind.
  • Reusing or not changing KwikPen needles. One new needle per dose, every time.
  • Not rotating sites, which invites the lumps described above.
  • A missed dose. There’s a specific rule for how long a window you have to take it late versus skip it — don’t guess, and never double up to catch up. Our missed-dose helper walks you through it.

Mild, short-lived redness, itching, or a small bump at the injection site is common and usually fades on its own. What deserves a call to your clinician: a site reaction that spreads, worsens, or doesn’t settle; signs of infection like increasing pain, warmth, or pus; or any symptom that feels bigger than the injection itself — a rash all over, swelling of the face or throat, trouble breathing, or severe belly pain that won’t go away. Those are not “inject through it” symptoms. When in doubt, see GLP-1 safety and contact your prescriber.

FAQ

Common questions about Mounjaro how to inject

Where is the best place to inject Mounjaro?

Any of the three approved subcutaneous sites works — the abdomen (at least 2 inches from your navel), the front of the thigh, or the back of the upper arm (⚠ verify against the IFU). None is more effective than the others; pick the one you can reach comfortably and rotate the spot each week. Avoid skin that's bruised, tender, hard, scarred, or reddened.

How do you use the Mounjaro single-dose pen?

The US single-dose pen is a push-button auto-injector — no needle to attach or see. You unlock the base, press it flat against the skin, and hold until you hear two clicks and the dose finishes (roughly 10 seconds). One pen delivers one weekly dose, then goes in a sharps container. Have your pharmacist or prescriber demonstrate the first one.

What angle do you inject Mounjaro at?

The single-dose pen is designed to sit flat against the skin, so there's no angle to judge — you press it straight down. If you use the multi-dose KwikPen sold in some markets, the Instructions for Use covers needle angle and whether to pinch a fold of skin. Either way, this is worth having demonstrated once in person.

Can you inject Mounjaro in the same spot every week?

No — rotate. Injecting the same patch repeatedly can cause lipohypertrophy, lumps of thickened fatty tissue that make later injections absorb unpredictably. Move to a different area each week and, within an area, shift the exact spot by an inch or so.

How long do you hold the Mounjaro pen against your skin?

With the single-dose pen, keep it pressed flat against the skin until you've heard both clicks and the dose window shows it's finished — about 10 seconds (⚠ verify against the IFU). Lifting it too soon can leave part of the dose behind. The IFU is the authority on the exact signal for your pen.

Does Mounjaro need to be refrigerated?

Yes — store pens in the refrigerator at 36–46°F before first use. A pen can sit at room temperature below 86°F for up to 21 days (⚠ verify window), after which it should be discarded. Never freeze it, and let a cold pen warm up a few minutes before injecting to reduce the sting.
